HOW IT WORKS
A parent fills out the first column during the drop off time. They indicate:

Whether the child will be in the nursery for one or both services

Then a parent hands the form to a nursery worker. This creates a positive touch-point and gives the parent a chance to express anything that they think nursery workers should know.
Nursery workers fill out column 2 and 3 during the course of the morning and give the completed form to the parent during the check out time.
